The Principal Investigator (PI) is required to sign the Research Data Use Agreement, F-00977. This contract describes data destruction terms under B.4. Immediately following the destruction of WCRS data, the PI agrees to contact DHS (preferably email DHSWCRSDataRequests@dhs.wisconsin.gov) with a written declaration executed by an authorized representative of recipient institution that identifies the study by name, date WCRS data have been destroyed and the method used, as well as confirmation that no individual-level information, data, or copies were retained in the possession of the research entity.

Registry fee structure summary

All research studies will be billed a one-time $800 application processing fee, and $100 per hour for analyst time. Patient-contact studies are subject to an additional $3,000 data release and ongoing study management fee. Any patient contact studies that exceed 30 hours of WCRS staff time on the project are also subject to the $100 per hour for analyst time. Linkage studies have a sliding scale fee dependent on cohort size
