**Note 1:** Physician statement of Gleason tertiary pattern can be used to code this data item when there is no other information available. **Note 2:** If present, a high Gleason Tertiary Pattern appears to be an indication for a worse outcome. **Note 3:** Record the tertiary pattern documented on radical prostatectomy or autopsy only.
